Most Indonesian households dispose of unused and expired medicines in household waste or drains, risking environmental contamination and antimicrobial resistance. Community pharmacists are well positioned to educate the public, yet they face constraints: materials not adapted to residents' language, no medicine take-back point, and limited English for Pharmacy Practice (EPP) skills for reading labels, leaflets of imported medicines, and English-language disposal guidelines. This community service program aimed to improve the EPP competence of pharmacists and pharmacy technicians, residents' knowledge and practice, and the capacity of community health cadres, and to establish a medicine collection point. The program ran from October 2026 to September 2027 at Aulia Farma Pharmacy and Mannuruki Subdistrict, Makassar, in five stages: preparation and baseline survey, bilingual media development, pharmacist and cadre training, three resident education sessions with a drop box, and monitoring-evaluation. Pharmacists' EPP scores (n = 3) increased by 38.2%, and all participants could explain English guidelines in plain language. Residents' knowledge scores (n = 30) rose from 52.4 to 81.6 (p < 0.001), and correct disposal practice increased from 23.3% to 76.7% at month three. The drop box collected 212 packages (9.8 kg), including 36 antibiotic packages, and 12 cadres were appointed as Medicine Disposal Agents. Combining language strengthening, participatory education, and a take-back facility effectively changed residents' knowledge and behavior.
